Transition Services

Thompson School District's Transition Services provide meaningful opportunities that foster confidence, self-sufficiency, and success beyond high school.

We support students with disabilities as they move from high school to adulthood, helping them develop the skills needed for independent living, further education, and competitive employment.

Who We Serve

Transition Services are designed for students ages 18-21 who receive support through TSD Exceptional Student Services and have transition goals outlined in their Individualized Education Plan (IEP). To ensure a smooth transition into adulthood, these students may need additional instruction and experiences in daily living skills, job readiness, and community access.

What We Offer

Thompson School District provides two specialized transition programs to help students gain real-world experience and develop essential life and employment skills.

Community Connections

Community Connections focuses on adult living skills, employment and volunteer experiences, and community access. This program helps students learn about personal finance, workplace behaviors, public transportation use, and self-advocacy through hands-on learning in the community.

Project SEARCH

A one-year, immersive school-to-work program based at Banner McKee Medical Center, where participants gain transferable job skills through workplace rotations, mentorship, and classroom instruction, preparing them for competitive employment.

School to Work Alliance Program

A collaborative initiative between the Colorado Division of Vocational Rehabilitation (DVR) and local school districts, the School to Work Alliance Program (SWAP) provides case management and direct services to young adults with disabilities who have mild to moderate employment needs. The program assists with vocational awareness, career exploration, and real-world work experiences to secure competitive, integrated employment.

Our programs offer individualized support, hands-on experiences, and collaboration with local businesses and organizations to ensure students are well-equipped for their futures.
